Young adults with IBD eventually transfer from pediatric to adult-centered care. The ImproveCareNow Transfer Toolkit is a 10-page document co-created by the Transition-Transfer Innovation Community, reviewed by the Patient Advisory Council and Clinical Practice Committee.

Why transition planning matters

Healthcare teams want young adults to navigate the system independently. Transition should feel seamless and happen when a patient is physically and emotionally stable, not only because they aged out of pediatrics.
