Belly pain scale

0 = no hurt · 5 = medium · 10 = worst tummy hurt. Caregivers can help younger kids pick a number.

Medicine (optional)

Skip if you prefer. A light “taken / skipped / N/A” note is enough here. Keep the full med list on visit prep.

Do you need to track medication? Not required for every meal. Optional fields help spot patterns (for example, missed dose on a school day). Use visit prep or your pharmacy list for the complete medication record.
